It’s absurdly difficult and unlikely to fatally overdose on ket alone. You’d probably have to be doing tens of grams at a time and/or injecting large amounts.
In the rare cases of deaths involving ketamine, they’re usually either from accidental or consequential effects, eg choking on vomit after passing out, drowning in a bath, or falling down stairs; or with other drugs having also been taken at the same time.
